COVID-19 Health & Safety verification

The health and safety of our staff is critical.  You are asked to complete this form DAILY prior to entering the Lucky Dog facility. If you answer "yes" to any of the verification questions STAY HOME.  You will be contacted regarding mandatory COVID-19 swab testing and the required quarantine time frame.

Note: Employees will be required to obtain a COVID-19 swab test and valid test results prior to returning to work, including a doctor's release. Test results may take up to 4 -5 business days.

Do you currently have any signs of being sick? (e.g., chills, loss of taste/smell, headache, nausea, shortness of breath or cough?
Do you have a fever or have you had a fever in the last 3 days?
Have you tested positive for COVID-19?
Have you been tested for COVID-19 and the results have not yet been confirmed?
Has any member of your household been sick, had a fever or test positive for COVID-19?
